The NTSB describes the issue partly as ‘pervasive ORGANIZATIONAL FAILURE’. The Enbridge organization structure did not have, in their organizational structure , a function that reported infrastructure integrity issues DIRECTLY to the Board….on a ‘need-to-know’ basis !
Would the equivalent of the ‘external audit’ practice,(say a ‘corporate internal infrastructure integrity audit’) where the shareholders and/or investors are given an INDEPENDENT overview…to protect THEIR interests…Boards typically don’t deal with ‘nuts and bolts’ issues..like ‘corrosion fatigue cracks’ ! Has the time come where pipeliners must be assured that they know ..and provide timely directives to remedy the situation !..
Not knowing is NOT AN ACCEPTABLE EXCUSE !
Or do we dither until some government decides that the equivalent of the accounting industry ,SORBANES-OXLEY act is required ?
